Testing

Homebound individuals may call (305)-614-1716 (Monday - Friday from 8:30 a.m. to 5:00 p.m.) to schedule a homebound COVID-19 testing appointment. If a homebound individual is receiving home health services or is a resident of a group home, adult living facility, nursing home or other congregate care facility, they should contact their provider to request an at-home test.

*NOTE: This is not a full list of COVID-19 testing locations. Check with your health care provider or the Miami-Dade County Department of Health at 305-324-2400 for a more comprehensive list of testing sites.

Vaccination

Homebound individuals 12 years of age or older can call (888) 201-5490 or email vaccinations@ilshealth.com to schedule an appointment at home to be vaccinated.
