Applicants Need:

Proof of Citizenship (Original U.S. Birth Certificate, previous U.S. Passport, or Naturalization Certificate)

Proof of Identity (Driver’s license, U.S. Passport, official military or government employee identification card, school identification card)

One Passport Photo – 2 X 2 inches. (Photograph can be taken at our office for a $12 fee)

Two Checks or Money Orders: First check payable to “City of Coral Gables” in the amount of $25 (execution fee) for first-time applicants and children under 16 years of age; Second check or money order payable to U.S. Department of State for $110 (passport fee) if applicant is 16 years or older; or $80 (passport fee) for under 16 years of age.
